If you would like to initiate a voluntary withdrawal from Denison for medical, personal or other reasons, you must complete an Exit Form and schedule an exit interview. If you are requesting a withdrawal for medical purposes, you must also complete a Medical Leave Form and Health Care Provider Report prior to your meeting.
