Student Accident & Sickness Insurance Plan

(Plan is optional, but requires a waiver be completed by the deadline.)

2012-2013 Plan Information

All full-time students are automatically enrolled in the Student Accident and Sickness Insurance Plan.  The premium is charged only once per year and is not related to the mandatory Health Center Fee charged to all full-time students every semester. The premium is charged during the first semester of full-time enrollment for the academic year. This is the Fall semester for most students, including Fall off-campus-program participants.

Enrollment is automatic for all students unless the required waiver request is completed by August 24, 2012.  If you do not want the Student Accident and Sickness Insurance coverage, you must have completed an on-line waiver by August 24, 2012.

  • Coverage extends for the entire 12-month period beginning 8/10/12 and continuing through 8/09/13 — 24 hours a day — at home, at school, or while traveling, including vacations.
  • The cost for the 2012-2013 academic year is $520.

A Student Insurance Information Card will be placed in the student's on-campus mail box in October, if the student does not waive the coverage.  The student should carry the card with them at all times. It provides information necessary for filing a claim. In the event that medical attention is required, this card should be presented to providers at the time services are rendered. Additional cards will be available in Student Accounts.

Claim Procedure

  1. If the only medical treatment rendered was from the Student Health Center, no claim form is required. Simply ask Student Accounts to submit that bill to the Student Insurance Plan. The student can do this by calling x6447 or by sending an e-mail to Beth Thorpe using your Denison email account. You should retain the itemized bill from the Student Health Center for your records.

    Please do not send these bills directly to Commercial Travelers Mutual Insurance Company. Student Health Center bills will be posted to your student account, as will any subsequent payments from the Student Insurance Plan.

  2. If you require medical treatment from a provider outside the Student Health Center

    • Present your Student Insurance Information Card and ask that an itemized bill be sent to:

    • It is imperative that you also complete and submit a claim form.
      You may pick up a claim form in Student Accounts, or you may download a claim form [pdf]. For 2012-2013 claims, use Policy Number 2012-M3A97. (For 2011-2012 claims, use 2011-M3A97.)
