Medical Excuses & Confirmations of Visit
Prolonged Medical Absence
When a medical practitioner determines that a student requires hospitalization or other medical accommodations, or the student is extensively absent from classes, the student can request that the medical provider or the Office of Health Services contact the Office of Student Affairs and/or professors. However, the student must provide permission or sign a release of medical information before any contact is made.
If You Miss Class For Medical Reasons
- E-mail your professors -- It is each student's responsibility to notify faculty if unable to meet academic obligations for any reason.
- At the time of the evaluation at Health Services, the student may request a "confirmation of visit" note. This note is to be shown to the faculty member who will decide if the student is excused from the academic responsibility.
