Accidental Death and Dismemberment (AD&D)
Summary
Insurance provides a benefit in case of accidental death or dismemberment.

None. Denison pay the full cost of this program.
Benefit
The death benefit is equal to 1.5 times an employee's budgeted salary. The death must a) be caused exclusively by external and accidental means; b) be the result of the injury, directly and independently of all other causes; c) occur within ninety days after you receive the injury.
Partial benefits are available for specific body dismemberments. Please refer to your summary plan description for more details.
Notes
Each employee names a beneficiary upon employment with Denison, which is kept on file in the Office of Human Resources. Be sure to keep your beneficiary designation card updated. You may change your beneficiary at any time.
This plan has been described here in a general manner. Your benefits are more fully described in the Plan document. In the case of a conflict or omission, the provisions of the Plan document will govern.
