Room Damage Billing Appeal Form
If you have been charged for missing items or damage on your year-end Room Condition Inventory for (RCI) and wish to appeal the charges, please fill out the billing appeal form to the best of your ability. All Billing Appeal Forms must be postmarked by June 24, 2011.
